Admission Criteria : New Visions Toronto’s Respite Visit Program serves adults who are medically fragile, physically and /or developmentally delayed. Individuals must reside in the City of Toronto. Families who want to access its respite program will go through the screening process prior to involvement:
1) Phone intake
2) Home Visit
3) Completion of application and information package
4) Tour of respite location
5) Short visit to meet residents and staff
5) Confirmation of dates for respite visit
Activities : Individualized supports according to each individual’s activity interests.
Comments : Due to the location and the fact that there is currently no charge for the visit, it is expected that the program will be utilized by many people. In order to accommodate visitors, visits may be limited.
