Admission Criteria : The priority focus of support will be for adults with complex physical and health care needs requiring a wheelchair accessible setting. IMPORTANT:Families who want to access the respite service should go through a screening process:
1.Pre-screening over the phone
2.Assessment for suitability
3.A Consumer Contact Information Package will be sent out and must be completed and sent back to Montage Support Services, attention to Mary Vieni, Project Coordinator.
4.Tour of the Respite Home
5.Completion and submission of medical forms
6.Confirmation of respite support for the individual
