Admission is made through a referral system.áPlease call Access and Information to request for an application package.áOnce the application is completed and returned to Sibley, a supervisor will contact the family to arrange for a preplacement visit.áIf you do not hear from the supervisor, please contact (416) 690-4130.Please note that offer of service is contingent upon assessment.
