If you are a family receiving Special Services At Home (SSAH) funding please call your coordinator for more information about the CHAP program and using your funds more flexibly. For a limited time SSAH coordinators will be able to assist SSAH families by inputting their application information onto the CHAP program. Click HERE for SSAH Coordinator contact information.
