Admission Criteria : | Caregivers supporting a child (0-18) with physical, developmental or mental health needs and living in Perth County may access coordination services for the purpose of respite. |
Activities : | The Respite Coordinator will work with caregivers to identify respite options and develop and implement a respite plan. |
Comments : | The Respite Coordinator will work with the family to identify the options that will work best for the person being cared for. Services may be provided by a volunteer, a special funding source (i.e. Special Services at Home, a service club) or purchased directly by the family. |
