General Information : Families who are caring for a child with a developmental and/or physical disability can apply to the special services at home program to get funding for primary caregiver relief and related supports or for activities of personal growth or development including camps and recreational activities, like swimming and music classes.

Passport is a program that helps adults who have a developmental disability be involved in their communities and live as independently as possible by providing funding for community participation services and supports, activities of daily living and person-directed planning. The program also provides funding for caregiver respite services and supports for primary caregivers of an adult with a developmental disability.
Eligibility : Ministry reviews eligibility using diagnostic information provided. Children (18 and under) with a developmental or physical disability with a developmental disability may qualify for SSAH program. Adults (18 and over) with a developmental disability need to contact Developmental Services Ontario to apply for Passport funding.
