General Information :
Access to all services at Meta Centre require families to go through an intake process. The referral of a potential client of Meta Centre services involves a telephone conversation and an intake interview with a member of the Social Work Services Unit.
The following information is required by telephone: 1) Personal:History/background 2) Family information 3) Medical information 4) Previous and current services.
Once the pertinent information has been recorded, an interview meeting will be scheduled to complete the intake process. During the interview, a client profile is completed and authorization forms are signed. The meeting includes the client, family members or guardians, and other external supports as appropriate.

10+ years with a primary diagnosis of developmental delay
